Why would you not provide the option for anonymous comments?
Hi Ellen — The form allows for anonymous submissions. There is no need to add your name/email address at the bottom. You can also submit the form multiple times, so you could share feedback anonymously, and then resubmit the form with your email address if you’d like to hear about future events. Hope that helps!
